Supported Housing

South Lane Mental Health offers a supported housing program that includes case management, medication management assistance, and individual skills training, as well as assisting clients in meeting their daily needs.

South Lane Mental Health has a multi-disciplinary staff of caring individuals who work together in a team approach, so that responsibility for client care is shared and coordinated. Staff disciplines include social workers, family therapists, nurse practitioners and other Qualified Mental Health Professionals and Associates.

    Housing options include:
  • A complex of subsidized apartments managed by South Lane Mental Health, with staff on site seven days a week, and emergency staff available 24/7;
  • Several community-based subsidized housing programs through agencies such as the Housing Authority and other non-profit organizations;
  • Section 8 housing; and
  • Free-market rentals in the community.
  • Case management services are delivered to any community site depending on need and desire of the client.

The goal of the supported housing team is to provide a safe, comfortable living environment, help clients reach their full potential, meet specific needs of individual clients, and encourage progress to more independent living arrangements.
